Student/Family Information Change
If information in your child’s enrollment record has changed (e.g., name, legal guardian, grade, etc.), please complete the Record Change Request.
If your child’s daycare or pick-up/drop-off location has changed or your child requires new or continuing transportation to and/or from school, please complete the Transportation Request or Change Form.
Change of Address
Proof of residence and photo ID will be required to make changes to a student’s address of record.
Contact the Welcome Center to determine the school located within the attendance area of your new address.

The Transportation Request/Change Form will need to be completed at the Welcome Center to transfer a student to the new school of residence.
An In-District Transfer Request may also be submitted to request that the student be allowed to continue at their current school. Should the In-District Transfer Request be approved, the family would be responsible for transportation to and from school.
